Discovery FCU Forms and Applications to Help you Manage your Account
Many of our forms and applications can be e-mailed to you for your electronic signature to save you time. To request a form or application, send us a secure message through online or mobile banking or call us at 610.372.8010.
You can also download, print, and complete the PDF forms linked below. The next time you’re in the neighborhood you can drop off your completed form at the credit union, or you can mail it to us at 2744 Century Blvd., Wyomissing, PA 19610.
Account Update Form
Update your name, address, telephone number, or e-mail.
ACH Origination Form
To establish a new ACH Origination from your Discovery FCU account, complete this form and return it to the credit union.
ACH Origination Change Form
If you need to change your current ACH Origination options with Discovery, please complete this form and return to the credit union. A 10-day time period is required to pre-note your account.
Close Checking Account Request Form
If you need to close your Discovery Checking Account, please print this form, complete your account information and sign the form where indicated. The credit union must receive your request to close your checking account in writing with the primary member or joint owner’s signature.
Close Savings Account Request Form
If you choose to close your basic savings account at the credit union, you will no longer enjoy all of the benefits of being a member at Discovery. The credit union must receive your request to close your basic savings account in writing with the primary member or joint owner’s signature. If you also have a checking account with the credit union, you’ll need to submit the Close Checking Account Request Form in addition to the Close Savings Account Request Form.

Use this form to assist you in balancing your checking account.
Remove Joint Owner Request Form (Use this form to complete & submit online)
If you want to be removed as a joint owner from a Discovery FCU account, the joint owner is required to complete and electronically sign the Remove Joint Owner Request Form.
2024 Skip-a-Payment Online Request (Use this online form to complete & submit your request online)
If you are a member in good standing, you may be able to skip two months of loan payments during the year 2024. ($35.00 processing fee applies per skipped payment)
2024 Skip-a-Payment Print & Return Request (Use this form to print, complete & mail back to the credit union)

Stop Payment Request
Stop payments on checks, electronic drafts, and pre-authorized electronic fund transfers.
Visa Balance Transfer Online Request (Use this form to complete & submit online)
Transfer your balance from a non-Discovery FCU credit card to your Discovery FCU Visa credit card and enjoy a low introductory rate of 2.99% APR for the first 6 months from the transfer date. After that, the APR will be your standard APR listed in your credit card application and agreement.
Visa Balance Transfer Request (Use this form to print, complete & mail back to the credit union)

Wire Transfer PIN Request
You are required to have a Personal Identification Number (PIN) to initiate wire transfers from your credit union account.
